HealthMark Group is hiring a remote Clinical Forms Completion Specialist to review medical information, complete FMLA and disability forms, process patient paperwork, and support HIPAA-compliant healthcare documentation workflows.

Job Description

The Clinical Forms Completion Specialist supports HealthMark Group’s health information management operations by reviewing, evaluating, and processing Disability and FMLA paperwork for patients seeking short-term leave, long-term leave, or family medical leave. This role gathers medical information, completes forms accurately, enters requests into internal systems, and helps coordinate return-to-work documentation.

This position is documentation-heavy and requires strong attention to detail, HIPAA compliance, medical office knowledge, and the ability to manage assigned caseloads independently. The role may include communication with patients, patient representatives, and medical professionals by email or phone regarding form status, missing information, and disability paperwork.

### Key Responsibilities

* Review and process Disability and FMLA forms
* Complete short-term disability, long-term disability, and family medical leave paperwork
* Gather medical information for assigned patient cases
* Communicate with medical professionals to collect required documentation
* Apply medical information accurately to support disability eligibility review
* Enter assigned requests into internal systems for tracking and processing
* Coordinate return-to-work transitions and restrictions when needed
* Answer patient questions by email or telephone related to forms
* Use standard responses to complete forms based on specialty practice type
* Verify patient information using key identifiers
* Follow HIPAA privacy and security requirements
* Ensure disability forms are completed within required turnaround times
* Manage assigned caseloads independently from start to finish

### Requirements

* Medical office experience required
* Strong written and verbal communication skills
* Strong attention to detail
* Professional and customer-oriented communication style
* Windows proficiency
* Ability to self-manage assigned work
* Ability to multitask and complete tasks timely
* Ability to maintain HIPAA compliance and patient confidentiality

### Preferred Qualifications

* Licensed Practical Nurse, Certified Nursing Assistant, Medical Assistant, or related certification preferred
* EMR experience preferred
* FMLA or disability forms experience preferred
* Healthcare documentation experience preferred
