Job Description

The Care Navigator supports Charlie Health’s admissions process by helping patients, families, and referral sources navigate access to behavioral health care. This role serves as a first point of contact for prospective clients and plays a key role in patient engagement, referral coordination, admissions support, and documentation.

The position is highly interactive and performance-driven, with responsibilities tied to admission rates, patient satisfaction, outreach efforts, time-to-admission goals, and referral outcomes. Care Navigators collaborate with admissions, clinical, outreach, and partnership teams to ensure clients and referral sources receive timely, supportive, and organized communication throughout the admissions process.

### Key Responsibilities

* Support clients and referral sources through the admissions process
* Communicate with patients, families, and external providers
* Understand patient needs, preferences, and barriers to care
* Make outbound referrals for individuals not admitted to Charlie Health
* Collaborate with internal clinical and admissions teams
* Document client and referral interactions in electronic systems
* Coordinate with Clinical Outreach and Partnerships teams
* Build knowledge of referral sources and available services
* Serve as liaison between Charlie Health and external partners
* Maintain a positive client and referral source experience
* Follow internal policies and procedures
* Meet admission, outreach, and performance metrics
* Support timely documentation and process completion

### Requirements

* Bachelor’s Degree in Health Sciences, Communications, Psychology, Social Work, or related field
* 1–2 years relevant work experience
* Healthcare experience preferred
* Patient-facing or customer-facing experience
* Strong interpersonal skills
* Strong relationship-building skills
* Strong listening skills
* Metrics-driven mindset
* Experience meeting KPIs or performance targets
* Excellent written and verbal communication skills
* Strong organization and attention to detail
* Salesforce proficiency
* Google Suite or Microsoft Office proficiency

* Work authorization in the United States
* Native or bilingual English proficiency
