Job Description

The Continuing Care Coordinator supports clients transitioning from Charlie Health programs by coordinating discharge planning, identifying aftercare resources, communicating with referral providers, and ensuring continuity of care. This role serves as a liaison between clients, families, providers, referral sources, and internal teams to ensure successful treatment transitions and positive long-term outcomes.

The position focuses heavily on relationship management, referral coordination, discharge planning, documentation, provider communication, and patient support. Success is measured through quality of care coordination, referral placement outcomes, documentation accuracy, and achievement of departmental performance goals.

### Key Responsibilities

* Provide treatment updates to referral sources
* Maintain relationships with external providers
* Coordinate discharge planning activities
* Identify appropriate aftercare resources
* Make referrals for clients completing treatment
* Utilize provider databases to locate referral options
* Communicate with referral providers regarding placements
* Follow up with clients and providers regarding referrals
* Verify placement and service availability
* Coordinate transfer of clinical information
* Ensure receiving providers have required documentation
* Document case management contacts and progress notes
* Communicate updates with therapists and treatment teams
* Collaborate with admissions, clinical, and outreach teams
* Build knowledge of referral sources and available services
* Maintain compliance with internal policies and procedures

### Requirements

* Bachelor’s Degree in Health Sciences, Communications, Psychology, Social Work, or related field
