Care Navigator

The Care Navigator supports patients attributed to Lifepoint Health’s Clinically Integrated Network by helping them access healthcare services, resources, and care coordination support. This role focuses on patient advocacy, preventive screenings, care plan communication, utilization support, and coordination across inpatient, outpatient, skilled care, hospice, home health, and primary care settings.

The Care Navigator works with patients, families, providers, payors, community resources, and internal teams to reduce barriers to care and support better health outcomes. This position requires strong communication, healthcare knowledge, care coordination skills, and the ability to work with diverse patient populations.

### Key Responsibilities

* Support high-risk and rising-risk patients
* Conduct patient outreach and engagement
* Complete preventive screenings
* Advocate for patient needs
* Connect patients with network providers
* Connect patients with healthcare facilities
* Coordinate payor-based resources
* Assist with prescription and DME-related resources
* Communicate care plans across interdisciplinary teams
* Coordinate continuity of care across healthcare settings
* Support primary care practices
* Help improve quality patient care
* Address barriers to care and service access
* Work with ACO and CIN stakeholders
* Support population health initiatives

### Requirements

* High School Diploma
* 2+ years ambulatory healthcare experience
* Ability to work independently
* Ability to prioritize care coordination tasks
* Ability to work in a team environment
* Microsoft Outlook proficiency
* Microsoft Word proficiency
* Microsoft Excel proficiency
* Microsoft Teams proficiency
* Strong communication skills
* Strong organizational skills
